CERVICOGENIC HEADACHE
Facts:
- Accounts for about 15% of headaches.
- May or may not be of traumatic origin.
- Medications may have only a marginal effect.
Symptoms: (see Cervical Facet Pain Maps)
- Unilateral headache, (rarely bilateral).
- Headache spreads from neck upwards (C3 or C2 distribution).
- Headache is non-throbbing.
- May be fluctuating or continuous.
- Patient may have dizziness, ipsilateral blurred vision, photophobia, phonophobia.
Diagnosis:
- May have abnormalities on cervical MRI scan.
- Provocation by pressing over occipital nerves.
- Provocation with neck movement.
- Decreased range of motion of the neck.
- Precision nerve blocks of the facet nerves and possibly nerve roots.
- Precision injections of the Atlanto-axial or Atlanto-occipital joints
Treatment:
- Precision facet nerve blocks.
- Nerve root blocks
- Radiofrequency rhizotomies of the facet nerves.
- Radiofrequency treatment of the dorsal root ganglion.
- Precision injection of the Atlanto-axial and Atlanto-occipital joints
